Objectives: Endodontically treated teeth (ETT) are commonly missing significant tooth structure resulting in the need for various restorative approaches including post and/or core placement. Current literature is divided on the impact of post and core restorations on ETT outcomes. Our study hypothesizes that post placement negatively affects ETT survival. This study aims to shed further insight on and to determine the effect of different types of intra-coronal restorations (ICR) on ETT survival.

Methods: This retrospective cohort study examined 230 dental electronic health records of patients who received endodontic and restorative treatment at Rutgers School of Dental Medicine between 2013 and 2016 with at least a 2-year follow-up. Average follow-up time was 53 months. Types of ICR evaluated included core buildup without post placement (n=107), cast post and core (n=48) and prefabricated post and core (n=75). ICR and additional factors affecting tooth survival, including patient demographics and presence of a crown, were analyzed with Cox Proportional hazard models at α=0.05.

Results: Age, sex, and the presence of a crown were statistically significant predictors of ETT survival. Survival rates for ETT were significantly lower in males and among older individuals (p < 0.05). ETT lacking a crown had a 4.3-fold higher likelihood of failure (p <005). The presence of posts did not affect the survival outcome.

Conclusion: Presence of a post did not affect ETT survival. Presence of a crown was the strongest indicator of ETT survival followed by age and sex.
