(PR001) Analysis of Internal Root Canal Anatomy Utilizing Optical Coherence Tomography: A Pilot Study

Objectives: Optical Coherence Tomography (OCT) is a non-invasive imaging modality utilizing near-infrared light to capture microstructural details of biological tissues. Previous studies have demonstrated OCT's potential for detailed internal visualization of root canals without radiation exposure. This pilot study evaluated OCT's ability to detect iatrogenic internal irregularities within single canal teeth.

Methods: Four single-rooted teeth, conventionally prepared and apically sectioned, were included in this study. Four lateral canals of varying sizes were created using Munce burs in each sample (#2, #1, #½, and #¼). Additionally, an internal resorptive defect was simulated in the coronal third of each root with a #2 Munce bur. A modified Dragonfly OPTIS Imaging Catheter, coupled with an integrated computer console, was used to create 3D images of the internal root canal anatomy. The samples were also scanned with Micro-computed tomography (Micro-CT). Lateral canal and internal resorption dimension were analyzed from the micro-CT and OCT scans with measurements repeated five times and analyzed utilizing a paired t-test (p < 0.05).

Results: There was no significant difference in the dimensions of the lateral canals using the Munce #2, 1, ½, ¼ between the OCT and Micro-CT images (p=0.57, p=0.61, p=0.21, p=0.15, respectively). Additionally, there was no significant difference in the dimensions of the internal resorptive defect between OCT and Micro-CT images (p=0.15).

Conclusion: In the current study, the images from the OCT and Micro-CT scans are comparable. With the advantage of no radiation exposure, OCT should be investigated further for chairside applications in endodontic therapy.
