(PR118) Histopathology of Endotontic Lesions and Their Correlation to the Radiographic Changes in the Maxillary Sinus Using CBCT Scans

This study examined the correlation between the histopathology of periapical lesions of endodontic origin and radiographic findings in the maxillary sinus observed through cone-beam computed tomography (CBCT). Materials and

Methods: A retrospective review was conducted on 39 patients with a preoperative CBCT who underwent maxillary SRCT with Biopsy results at university clinic. CBCT scans of the lesions were independently reviewed by an endodontic resident and a board-certified endodontist to identify the category of MSEO (Maxillary Sinusitis of Endodontic origin). Biopsy results were analyzed for histopathologic diagnosis and correlated to the category of MSEO.

Results: Among the 39 lesions, 65% showed changes in the maxillary sinus on CBCT scans, while 35% showed no changes. Of the lesions analyzed, 82% were granulomas, 15.5% were cysts, and 2.5% were odontogenic keratocysts (OKCs). Of the 26 lesions associated with sinus changes, 50% presented as periapical mucositis (PAM) and 50% as periapical osteoperiostitis (PAO).

Conclusion: The histopathology of periapical lesions did not influence the type of changes observed in the maxillary sinus. However, proximity to the sinus significantly increased the likelihood of sinus changes. These findings highlight the importance of considering lesion location in assessing potential sinus involvement.
