Objective(s): Evaluate outcomes of non-surgical endodontic retreatment (ReTx) performed by a single endodontist, in private practice.

Methods: Data from 202 patients who underwent ReTx in private endodontic practice between 1986 and 2019, with a minimum follow-up of 12 months, were evaluated. The Periapical Index (PAI) scoring system was used by two calibrated endodontic residents to evaluate pre- and post- operative periapical radiographs. CBCT assessment was also used to evaluate periapical healing. Interrater agreement (Kappa score) for PAI and CBCT evaluation was calculated. A third reviewer, dual-trained endodontist and radiologist, served as a tiebreaker in cases of disagreement. 617 radiographs from 241 cases were reviewed.

Results: Median patient age at time of ReTx was 54 years. Median follow-up time after ReTx was 10.4 years. Radiographic success was as follows (strict, loose respectively): at 5 years (85.5%, 90.5%); at 10.4 years (74.7%, 82.2%). Tooth survival was 93.4% at 5 years; 87.1% at 10.4 years. Kappa score for PAI assessment was 0.819, and a third reviewer was needed for 83 cases. Molar teeth had the lowest success and survival, compared to anterior and premolar teeth.

Conclusion: Non-surgical endodontic retreatment has favorable clinician- and patient-centered outcomes. ReTx outcomes in this study may be generalized to other endodontic private practices due to all data being collected from a single practice and practitioner. Next steps include ongoing CBCT evaluation, and comparison of Kappa scores from PAI and CBCT assessments.
